Nurse to join our team. The ideal candidate will have ICU and Ward experience, with expertise in caring
for tracheostomy and critical care patients.
Job Summary:
We are seeking a skilled and dedicated Home Care Nurse to provide high-quality patient care in a home
setting. The successful candidate will be responsible for delivering personalized care to patients with
complex medical needs, including tracheostomy and critical care patients.
Responsibilities:
- Provide high-quality, patient-centered care to patients with complex medical needs in a home setting
- Assess, plan, implement, and evaluate patient care plans in collaboration with the patient's healthcare
team
- Manage tracheostomy care, including suctioning, dressing changes, and troubleshooting
- Provide critical care services, including vital sign monitoring, medication management, and wound care
- Collaborate with other healthcare professionals to ensure comprehensive care
- Educate patients and their families on disease management, treatment options, and self-care
techniques
- Maintain accurate and timely documentation of patient care
- Participate in ongoing professional development to enhance clinical skills and stay current with best
practices
Requirements:
- Any state Registration with all education documents
- Minimum 1 year to 2 years of experience in ICU and/or Ward settings
- Experience caring for tracheostomy and critical care patients
- Strong clinical skills, with the ability to work independently in a home setting
- Good communication
- Reliable transportation and willingness to travel to patient homes
Preferred Qualifications:
- Bachelor's degree in Nursing or diploma in nursing
- Registration with proper work experience
